Volunteer Monthly Report & Stats Form (Revised)

Greetings,

The volunteer services monthly report & stats form, listed under the Form Section of the blog, has been updated to include the recently announced revision to the template.  Please ensure to review the updated document and properly note the badge number for active Houston Public Library volunteers.

The inclusion of volunteer ID numbers will help SPACES in monitoring badge activity.


If a volunteer is new and has not received a badge, serving on a temporary/short-term basis, or helping on a special event/project as part of  a group, please note with "N/A" (not applicable).
