New Badging Request Forms

Effective immediately, the badging office has developed and implemented a new request form for all employees, contractors, temp workers, and volunteers to utilize when obtaining proper identification. 

For Library volunteers assigned to the Central Library & departments within the Julia Ideson Building, an official access form will also be required to properly document volunteers who need entry to designated staff areas. 

Please refer to the most recent emails from volunteer services for the new badging request form and the consolidated access form. The new forms will serve as the sole documentation for purposes of identification.
